Soap Notes

A Soap Notes is required for every appointment. Complete a Cash Soap for out-of-pocket appointments and a Treatment Soap for all insurance appointments.

Progress Reports Notes

Progress Reports are sent to L&I Case Managers and Doctors for review. A Progress Report is required after appointment #6 and #12 for all L&I patients. Admin will notify you if a doctor needs a progress report for a patient.
