Labor & Industry Injury Claims
We accept L&I claims through the State of Washington and private companies. Please verify with your claim manager that your claim has been allowed and is open for billing before making your first appointment.

Before scheduling your first appointment, we will need all of the following information:

Name of your L&I company

Claim#

Date of your accident

Contact name and phone# of your Case Manager

A referral for massage therapy from your doctor, naturopath or chiropractor

A copy of your ID

A phone number to call you back to schedule appointments
Please email the above information to admin@lakestevensmassagetherapy.com. Your doctor may fax your referral to 425-377-9487.
Please keep in mind that all massage visits must be pre-approved by your Case Manager before scheduling. Once we receive a response after requesting an authorization, we will contact you to set up your first 6 massage visits.

How many appointments can I have?
You will be approved for six appointments at a time by your claim manager. After your first six visits, we can request a second set of six appointments if your doctor recommends further treatment and treatment is approved by your claims manager.
Do I have to have a pre-authorization?
Yes, all L&I massage treatments must be approved by your claims manager before your appointments are scheduled.
