Auto Accident Claims

We only bill first party insurance. This means that the PIP claim needs to be through your auto insurance policy and not the other party involved in the motor vehicle accident. We do not bill 3rd party PIP.

PLEASE be sure to monitor your PIP benefit to avoid overbilling your allowed coverage.

Before scheduling your first appointment, we will need all of the following information:

 

  • Name of your insurance company (we only bill your insurance, not the other party involved)

  • Claim# 

  • Date of your accident

  • Contact name and phone# of your Claims Adjuster handling your medical bills

  • A referral for massage therapy from your doctor, naturopath or chiropractor

  • A copy of your ID

  • A phone number to call you back to schedule appointments

 

Please email the above information to admin@lakestevensmassagetherapy.com. Your doctor may fax your referral to 425-377-9487.

 

Please keep in mind that it is your responsibility to keep track of your PIP benefit. We do not have access to your limits, amount already spent or outstanding balances in process. Any overbilled balances will be the responsibility of the client.

How many appointments can I have?

Your doctor or chiropractor will recommend the number of visits with your referral. We do allow for successive referrals on the same claim as long as your PIP benefit has not been exhausted.

What happens when my PIP benefit runs out?

It is your responsibility to keep track of how much of your PIP benefit you have spent. Claims denied due to exhausted funds will be billed to the client’s health insurance, attorney or to the client directly at $160 per visit.

What if I have an attorney?

We will be happy to work with your attorney as long as we have a signed Release of Information on file for you from your attorney’s office.

How long does it take to get my first appointment?

We will book your first appointment as soon as possible once we have confirmation from your claim adjuster that your PIP claim is open and billable.
