Health Insurance
We currently accept Aetna, Kaiser Permanente, Premera Insurance. Be sure to check with your plan to verify you have coverage for treatment from a licensed massage therapist and how many visits your plan allows per year.
While we do accept Regence, we are currently at max capacity and are not accepting new clients.
Before scheduling your first appointment, we will need all of the following information:

Insurance ID#: (including the letters in front of the numbers)

A referral for massage therapy from your doctor, naturopath or chiropractor

A copy of your insurance card and ID

A phone number to call you back to schedule appointments
Please email the above information to admin@lakestevensmassagetherapy.com. Your doctor may fax your referral to 425-377-9487.
Please keep in mind that while we can see what your copay will likely be, we do not always get specific benefit information for massage therapy on your plan. Therefore it is your responsibility to understand the insurance coverage for massage therapy on your health plan.

Why do I need a referral for massage therapy?
Our therapists do not diagnose for any conditions or injuries. In order to bill your insurance, we are required to provide them with a diagnosis code for treatment provided. We get these diagnosis codes from your referral. A referral can come from your doctor, naturopath or chiropractor.
Do you bill secondary insurance?
We do not bill secondary insurances.
What if I still haven't met my deductible?
If you still owe a deductible, we will send you an invoice after we have billed your insurance company.
How much will my appointment cost?
The cost of your appointment will be determined by your plan's copay or coinsurance requirements.
